Subject: RSI Update
1. Topic-
Rapid Sequence Intubation changes and updates to medications and Procedure

2. Content-
Lecture regarding recognizing respiratory failure and immediate need for Endotracheal Intubation. Detailing the sequence and medications used to complete procedure.

3. Goals: Aims/Outcomes-
1.To provide refresher education in RSI for the credentialing process.
2.To explain the function of the medications used to ensure competency.
3.To raise awareness for the importance of recognizing and managing respiratory failure quickly and definitively.

4. Objectives-
1.List four assessment conditions that indicate the need fore endotracheal intubation
2.Describe the concept and origin of Rapid Sequence Intubation
3.Define Functional Residual Capacity
4.Describe the difference in mechanism of action between depolarizing and non- depolarizing neuro-muscular blockers.
5. List 4 contraindications to Succynlcholine.
6. List failed airway options for R.S.I according to SHCA Protocols.
7. Define the rule of five "P"s
8. List the meds used for sedation, their onset, duration, and order used in the R.S.I. procedure.
9. List the medications used for paralysis/ their onset, duration, and indications for use.
10. Describe the R.S.I. sequence in order and describe the Sixth"P"

5. Materials and Aids-
Power Point Presentation. Medication dosage spreadsheet handout. Student quiz. Evaluation form

6. Procedures/Methods-

A. Introduction-

1. The Origin of R.S.I.
2.The High Incidence of misplaced ET Tubes on delivery to ER
3.How to recognize respiratory failure

B. Development-

1. Functional residual capacity defined.
2.premedications for special circumstances, Lidocaine and Atropine.
3.compare and contrast non depolarizing paralytic agents with succynlcholine.
Discuss indications for each type and contraindications for Succynlchholine.
5.List medications used for sedation, their class and mechanisms of action and dosing information.
6. List proper dosing, onset of action and duration of each of the paralytics used.
7. Describe appropriate failed airway devices from opa to supra-glottic airways.
8. Describe proper confirmation of tube placement and documentation.
9.Describe proper sedation maintenance per protocol and the appropriate time to notify medical control.
10. describe proper handling techniques of a patient who is intubated.

C. Practice-

1.In the skills portion of the class, Student will see a demonstrated RSI scenario on the mannequin.
2.Students will then practice being the primary Paramedic responsible for 2 other Respiratory failure scenarios

E. Accommodations (Differentiated Instruction)-

Classroom setting with overhead projection for the power point presentation and suitable classroom environment for skills practice and testing with a mannequin simulator

F. Checking for understanding-

1.Students will utilize an audience response system to actively answer questions during the presentation
2.Students will be assessed for non verbal cues during the lecture that they are actively listening and participating in the discussion.
3.Questions will be posed to the students throughout the presentation to emphasize student comprehension of presented material.

G. Closure-

The procedure itself will be reviewed at the end with just the steps needed to take and the correct order in which to take those steps so that the proper application is fresh in their minds at the conclusion of the lecture.

7. Evaluation-
1.The Students will complete a written Quiz at the end of the lecture portion of the presentation
2. The student will complete a skill evaluation of a simulated respiratory failure case on a realistic mannequin as the lead provider.
