3. Goals: Aims/Outcomes-
|
1.The trifold will demonstrate 100% compliance in utilizing non
clinical or scientific words or explanations. (Cognitive)
2. 100 percent of the information will be reformatted in to a bookmark
design with information described in bullet points and illustrations/diagrams.
(Psychomotor)
3. The trifold will demonstrate 100% compliance in utilizing words
under three syllables and short sentences. (Affective) |

4. Objectives-
|
1. The trifold will define the information needed with language
common to the general population at or below the 6th grade reading
level.
2. The trifold design will reflect a bullet point organization and
illustrations/diagrams rather than paragraph organization.
3. The trifold will modify language that is appropriate for the targeted
patient population to comprehend, generally below the 6th grade reading
level. |

6. Procedures/Methods-
|
A. Introduction-
1.Most women are unaware of the side effects from breast cancer
treatment.
2. Most women are unaware that physical therapy interventions may
improve their quality of life.
3. Most women are not informed as to where to seek treatment. |

B. Development-
1. There is a lack of information in the general public regarding
the complications resulting from breast cancer treatment.
2. TurningPoint Women's Healthcare offers treatment and programs to
assist with recovery from breast cancer.
3. A trifold handout is available for initiating patient awareness
and education. |

|
C. Practice-
1. The therapist will explain the bullet points of information in
the trifold to a single person in the target audience.
2. The therapist and the single person will discuss the bullet points
in the trifold.
3. The single person will paraphrase the bullet points to the therapist. |
|
|
D. Independent Practice-
1. The single person will determine if she is in need of therapy
interventions.
2. The single person will assess if she knows of one other person
who may be in need of the interventions.

|
E. Accommodations (Differentiated Instruction)-
1. The therapist will read the bullet points to the patient is the
reading level too low to comprehend.
2. The trifold will be translated into Spanish.
3. The trifold will be printed in larger font (16 or 18) to accommodate
visual impairments. |

F. Checking for understanding-
1.Prior to leaving the session, the single person will be able to
recite 3 bullet points.
2. The patient will respond with 80% accuracy to a simple survey created
by the therapist regarding the information in the trifold.
3. The single person may be able to identify several friends that
are in need of the service. |

G. Closure-
1. The therapist will reinforce the need for women to seek additional
assistance during and after breast cancer treatment.
2. The therapist will identify the physical therapy clinics that offer
this specialized care. |

7. Evaluation-
|
1. The single person gives 80% return response to information discussed.
2. The single person makes an appointment for an assessment or recommends
a friend make an appointment for an assessment. |

8. Teacher Reflection-
|
Time limits need to be in place to stay within the constraints of
an appointment hour. Follow up appointment should be offered and available.
More diagrams and illustrations need to be added to the trifold. |
