Set Goal for phase 1 of project. Duration of phase 1 is 10 weeks.
State area of physical fitness selected for focus.(muscular strength, muscular endurance, cardiorespiratory endurance, flexibility or body composition.)Example: long term goal is to increase muscular endurance
State the method of evaluation of progress toward the long term goal (i.e.: muscular endurance progress will be measured periodically by evaluating the number of push-ups done in 1 minute.)
State the specific goal to be acheived by the end of the project (i.e. increase number of push-ups done in 1 minute from 13 to 25.)
Document the reason this goal was selected and how it relates to overall improvement in wellness.

Conduct research and compile data to evaluate in developing the prescription for acheiving the goal.
Compile data from Concepts of Wellness textbook, "Total Fitness and Wellness." relevant to the selected goal. Summarize results of research.
Conduct additional research such as magazine articles related to selected goal. Summarize results of research.
Conduct additional research, including but not limited to results of studies related to selected goal. Summarize results of research.
Conduct additional research, including but not limited to interview with health and fitness professionals, relative to the selected goal. Summarize results of research.

Document progress on a weekly basis.
Using the data compiled during research phase, develop a weekly prescription to build to final goal.
Include a mini-goal for each week.
Document whether or not plan for the week was followed, and whether or not mini goal was met.
If plan followed and mini goal met, document any behavioral changes that were necessary to facilitate success and set new mini goal for next week. If plan was not followed or mini goal was not met, analyze the reason and document plan for overcoming the obstacle(s) in the following week and determine if same goal appropriate for following week or modify.

Set goal for phase 2 of project (nutrition.) Duration of phase 2 is 5 weeks (4 weeks for Saturday students.)
Using the results of your lab data from chapter 7, determine area of nutrition to focus on for improvement. (i.e. calories, fat, fiber, protein, carbohydrates, hydration, sodium, food group balance.)
State reason(s) why this particular goal was selected and how acheivement will improve overall wellness.

Overall Evaluation: final measure of progress for both phases, state whether or not each long term goal met, document measure of progress (i.e. phase 1# of push-ups done during final week.)(i.e. phase 2: number of servings of fruit/vegetables eaten on a daily basis.)Recap behavioral changes made to overcome obstacles.
